For all patients: Please bring your Alberta Health Care card to all visits. (A photo ID is also required for insured AB Health Care services.) Please bring a list of your medications, or any forms that you want the doctor to fill.
In accordance with the Alberta Health and Information Act, reprints of invoices, receipts, and completion of forms and documents may have extra applicable fees.
If you have alternate eye exam coverage from the government (IA, SS, AB Works, etc.), please notify our office by email or phone call, prior to the eye exam date, for a pre-approval prior to your visit.
Insured Services by Alberta Health Care
All patients under the age of 19 and over the age of 65 are covered for a routine annual eye exam. Extra tests that are not covered by AHC, like retinal photos, may incur extra fees. For other fees not covered, please ask the doctor directly.
All patients with valid Alberta Health Care coverage, regardless of age, are now covered for any urgent or medical visits, eye health care, or emergency care visit at our offices.
The coverage for medically necessary eye health services includes:
- Monitoring for diabetes related issues, glaucoma, retinal diseases
- Pre and post operative optometric care for cataract patients
- Eye infection or injuries
- Removal of foreign objects from the eye
- Sudden changes in vision
- Treatment of eye infections, inflammation, or injuries
Please inform the doctor in advance of the nature of your vision disturbance to ensure it will be covered.
The medical eye health exam visits do not include routine yearly eye exams, or annual exams for RX (refraction) for prescriptions for eyeglasses or contact lenses. Only medically necessary eye health check visits are covered under Alberta Health Care.
You are also covered with a referral from your doctor. Patients with a referral letter from their physician for medical issues also have full coverage for the ocular health check up services requested through their Alberta Health Care coverage.
All Alberta Health Care insured services require a valid AB Health Care insurance card and a photo ID.
Uninsured Services
- Complete Eye Exam: confirm with your doctor (doctors accept cash, debit/Interac, or Mastercard only. For patients 19–64 years old, or patients not insured by Alberta Health Care.) Includes case history, external eye exam, internal eye exam, ocular health check, eye pressure test, HD retinal camera capture if medically needed, vision tests, refraction and glasses prescription if needed, diagnosis and recommendation.
- Follow Up Services: confirm with your doctor, as services and tests required may vary. For current patients that need follow up care, and patients 19–64 years old or patients not covered by Alberta Health Care.
- Forms Completion: $10 to $40 (for drivers forms, RCMP/police forms, employment forms, etc.)
- LASIK Surgery Pre-Op and Post-Op: confirm with your doctor (includes all required visits and forms)
- Punctal Occlusions: Temporary plugs (last 3–12 months) confirm with your doctor / Permanent plugs confirm with your doctor
- Reprints of Lost Receipts: $15 (will display "duplicate", copies only, not original numbered)
- Rechecks of RX Prescription: A fee may be applicable if your last recent eye exam was more than 3 months ago, or if your eyeglasses were not dispensed by an Alberta licensed optician or optometrist, or if you see a different doctor than the original doctor. Please ask the doctor first.
If you experience difficulty with your new RX, it may be due to a variety of factors: medical issues, RX fluctuations or changes, fit, wear, glasses, type or brand of lenses, distance of reading or computer, etc. We recommend you visit the Optical where you bought the glasses first to ensure the RX order, lenses, fit, etc. are accurate to the RX ordered. Our doctors use the latest instruments and technologies to diagnose medical or RX problems. If you need to see the doctor for verification or recheck of RX, please book the recheck of RX appointment with the same doctor as before, as the doctor wants to ensure other issues are ruled out and that you have an accurate RX.
We cannot provide refunds for medical visits or services that have already been rendered. As per College by-laws, our doctors are independent from the Optical or Costco: our doctors do not directly or indirectly share profit for glasses, contact lenses, or any other products sold, and Costco does not directly or indirectly pay the doctors or pay for any other expenses. Prescription (RX) Reprints
Our doctors will always give you your prescription and receipt at the completion of your eye exam. However, if you later lose or misplace your RX and want another copy, in accordance with the Alberta Health Information Act, we need to verify your identity and record your request, to ensure your personal health information is not delivered or released to the wrong person.
In order to verify your identity, we need:
- Your complete name, first and last, as spelled in the medical chart previously.
- Your date of birth (as we can have many different patients with the same name).
- The approximate date and location of your last eye exam.
- The fax number where you want it sent, including the area code.
- Your phone number or contact info, in case we need to verify or confirm information (if it differs from the medical chart file).
- If the request comes from someone other than the patient (e.g. another optical or doctor's office), a signed release consent form, signed by the patient.
Please allow time to complete your request, as the doctor usually has to verify the medical chart before re-issuing the RX.
Receipt Reprints
Due to accounting and insurance fraud prevention requirements, we can only issue one original copy of the receipt, provided at the completion of your eye exam. If at a later date you require another copy displaying "duplicate," there may be extra fees. The same ID information described above is required.
Email Requests
If you prefer to have your RX copy or receipt emailed to you, or if you have any other health questions or requests via email, we must ensure your email is valid and belongs to you, that there is no error in the address, and that we have your clear authorization to email your health information.
To process the request, please first email info@drquan.ca with the following information:
- Your complete name as spelled in the medical file.
- Your date of birth.
- The date and location of your last eye exam.
- Your clear, express authorization that you want us to email (unsecured) your medical data back to you.
Only once we receive this can we reply to your email with the requested information and send your private health information by email.
Phone & Record System
As required by the Alberta Health Information Act and the standard of care in optometry, we must properly record every patient request to ensure protection of your identity and personal health information, and to ensure timely processing of your request.
Requests can be made in writing (email, fax) or by leaving a recorded voice message in our phone system. Every message we receive is properly recorded and archived, this ensures no errors are made and that we have proper consent to act on your behalf (for example: faxing to a specific number, releasing information to a different provider, changing a referral, or making, cancelling, or changing appointments).
Insurance Audits & Requests
From time to time, your insurance company may conduct a random audit and verify your claims. As insurance companies are not health care providers or health information custodians, we cannot release any information or confirm questions about services provided without a properly signed release consent form, signed by the patient. Please ensure your insurance company follows the proper procedures for claims verification.
Insurance Claims & Verification
After your eye exam, the doctors will give you a unique numbered receipt and RX containing all information required by the College of Optometrists and accounting requirements. Our doctors are required to sign and provide patients with an RX and receipt that are compliant with all regulations. Our receipt is a standard receipt widely accepted by all insurance companies.
Because each insurance company may have different claims protocols, we cannot reissue receipts in a different format for a specific insurer. Unfortunately, some insurance companies have started asking patients additional questions or demanding further clarification, even when all required information is already on the receipt or RX. Below are answers to some of these common requests:
